Absence Excuse Form

A form to inform your Conductor if you will be late to or absent from a rehearsal

If you are going to miss all or part of a rehearsal for any reason, you must inform your division's Conductor.

Fill out and submit this form (it'll be emailed to your Conductor)
OR pick up a form at rehearsal,
fill it out and hand it your Conductor in person.
