Here are the instructions and forms you'll need to register for BYSO in the 2008-09 season.

Please read this page carefully - our registration forms have changed this year! 
You can now register online, which will save a lot of data entry in our office - thanks.

Before your Audition:

1. Please complete and submit the general Registration Form online (at least 24 hours before your audition)
   
2. Please download, print and complete the Medical and Release of Liability Form EXCEPT THE SIGNATURES - you must sign the form in person at the Audition.

At the Audition:

1. You will need to complete an Audition Details form about your playing experience for the conductors.
2. You (both student and parent) will need to SIGN a Student Contract, a Photo Release and the Medical and Release of Liability Form.
3. We will give you a School Participation Form, which requires your school music teacher's signature.
